ESM Cares: Services

Celebrating 13 years of service to the Washington community

Information and Assistance:

  • Provide information on community resources for seniors by telephone.

Assessment and Care Planning includes:

  • Home visit to interview the senior and meet with family members.
  • Comprehensive assessment evaluating the senior's physical and mental health, their support network, economic resources, insurance coverage and their home environment.
  • Detailed written evaluation and care plan tailored to match the senior's individual needs with available resources.
  • Personal consultation to discuss the evaluation and care plan

Care Management Services:

  • Provide initial care plan implementation
  • Monitor in-home services to ensure continued quality and appropriateness.
  • Counsel seniors and families during periods of stress, transition and illness.
  • Coordinate support services following a return home after a hospital, rehabilitation or nursing center stay.
  • Assist seniors who want to move from home to an assisted living or nursing facility.
  • Facilitate communication among family members and between or client and health care providers.

Who Can Request Services?

Seniors living in the Washington area, Montgomery and Prince George’s Counties and family members or other caregivers of a senior who lives in these areas.

Service Fees:

Services are offered on a fee basis. Subsidies are available based on monthly income. These subsidies help to enable all area seniors and their families to access these services.

Typical Examples:

An 88-year-old widow was able to remain in her home after returning from the hospital with the coordination of heavy duty cleaning, home health aides, home delivered meals, and transportation to the doctor's office

A family was able to find a new residence for their older relative that provided the support and service they needed within the price range they could afford.

...are living in or out of the area and need help identifying, coordinating, and monitoring their family member's care.
